Include: 1. name of the job 2. company 3. date posted 4. job application URL Then, find the best contact person I can reach out to in order to introduce myself and let them know I have applied. Find the best email address for them. Write a brief introduction I can send them. It should include an introduction of myself and why I'm reaching out, mentioning the specific role.
